Clients are responsible for maintaining their own health records and documentation.
If you choose to share personal health information during a consultation, it is used only for the purpose of that session and is not retained or stored afterward.
Because we do not store or manage PHI, this service does not function as a HIPAA-covered entity medical practice, and no ongoing medical record is created.
We encourage all clients to:
Maintain copies of their medical records.
Keep personal documentation of medications, test results, and provider visits.
Share relevant records directly with their licensed healthcare providers as needed.
