Scope of Services

Colorado RN Virtual Mental Wellness Support Services

The mental wellness support services provided through this practice are offered by a Registered Nurse (RN) licensed in the State of Colorado and are delivered in accordance with the Colorado Nurse Practice Act

These services are educational, supportive, and wellness-focused in nature. They are designed to help young adults, midlife adults, and older adults better understand stress, build coping skills, improve emotional resilience, and strengthen overall well-being.

These services are not psychotherapy, counseling, psychiatric treatment, or medical diagnosis.

If you require psychotherapy, psychiatric care, or mental health diagnosis, you will be referred to a licensed mental health provider (such as a Licensed Professional Counselor, Psychologist, or Psychiatrist) in Colorado.

Emergency & Crisis Disclaimer

This service is not appropriate for individuals experiencing:

• Suicidal thoughts

• Thoughts of harming themselves or others

• Severe depression or psychiatric instability

• Mental health emergencies

Telehealth Notice (Colorado)

Virtual sessions are conducted via secure Google telehealth platforms. While reasonable safeguards are used to protect your privacy, electronic communication carries inherent risks. By participating in services, you acknowledge and accept these risks.

Client Acknowledgment

By engaging in services, you understand and agree that:

• This is an RN-led wellness and educational service.

• It does not constitute therapy or psychiatric care.

• Participation is voluntary.

• You are responsible for seeking appropriate medical or mental health care when needed

• Services will be provided virtually via a secure telehealth platform.

• Telehealth involves electronic communication and may include video, phone, messaging, or document sharing.

• While safeguards are in place, electronic transmission carries inherent privacy risks.

• I may withdraw consent for telehealth at any time.

• I am physically located in the State of Colorado at the time of service.

• The RN providing services is licensed in Colorado.

I understand this service is educational and supportive in nature and does not include psychotherapy, diagnosis, or medication management.

I understand that in the event of a medical or psychiatric emergency, I am responsible for calling 911, 988, or seeking emergency care.

By engaging in services, I understand and agree:

  1. Services are wellness-based and educational.

  2. The RN does not diagnose or treat mental health conditions.

  3. The RN does not prescribe medications.

  4. This service does not replace therapy or psychiatric care.

  5. I am responsible for my own medical and mental health decisions.

  6. I will inform the RN if I am under the care of a mental health professional.

  7. I understand that referrals may be provided if care exceeds RN scope.

I agree to participate voluntarily and take responsibility for implementing recommended wellness strategies.
